A Boca Raton restaurant was ordered closed by the Florida Department of Business and Professional Regulation after rodents, flying insects, and “objection odors” were documented.

BOCA RATON, FL (BocaNewsNow.com) (Copyright © 2024 MetroDesk Media, LLC) — Cafe Lavie in Boca Raton was ordered closed by the Florida Department of Business and Professional Regulation on September 5th after a health inspector allegedly found 13 violations — including rodents and flying insects. The restaurant at 5030 Champions Gate Blvd. has since been allowed to reopen.

Per BocaNewsNow.com policy, the following is an unedited list of what the health inspector documented in the restaurant during the inspection:

High Priority – Rodent activity present as evidenced by rodent droppings found. Observed approximately 30 + rodent droppings on top of ware washing machine in ware washing area in kitchen. Observed approximately 5 rodent droppings inside of bin containing wrapped single service items on cook line in kitchen Observed 4 rodent droppings on ground on cook line in kitchen. Observed 1 rodent dropping on ground at front counter in front of ice machine **Warning**

High Priority – Raw animal food stored over or with ready-to-eat food in a freezer – not all products commercially packaged. Raw chicken stored above cooked broths in reach in freezer **Warning**

High Priority – Operating with an expired Division of Hotels and Restaurants license. License expired 12-1-23 **Admin Complaint**

High Priority – Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 2 live small flying insects at front counter in dining room not landing. Observed 3 live small flying insects landing on clean wares on cook line in kitchen Observed 7 live small flying inspects hovering at prep table holding slicer on cook line in kitchen. Observed approximately 25 live small flying insects hovering and landing on walls/boxes at kitchen entrance in kitchen **Warning**

Intermediate – Spray bottle containing toxic substance not labeled. Spray bottle containing clear substance not labeled. Operator labeled sanitizer **Corrected On-Site** **Warning**

Intermediate – Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Broths prepared and frozen not redated when held at refrigeration temperatures again. Operator redated **Corrected On-Site** **Warning**

Intermediate – No measuring device available for measuring utensil surface temperature when using hot water as sanitizer in a dishmachine. Operator unable to provide at time of inspection **Repeat Violation** **Admin Complaint**

Intermediate – No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.

Intermediate – Food-contact surface soiled with food debris, mold-like substance or slime. Cutting boards stained, soiled **Warning**

Intermediate – Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Milk opened 2 days ago per operator to date marked **Warning**

Basic – In-use utensil stored in standing water less than 135 degrees Fahrenheit. Utensils in standing water of 74F. Operator removed utensil and discarded water **Corrected On-Site** **Warning**

Basic – Objectionable odors in ware washing/ dry storage/areas of the establishment. **Warning**

Basic – Hole in or other damage to wall. Observed at wall next to pho station **Warning**

The restaurant was reinspected on September 6th and permitted to reopen. This was the first ordered closure of Cafe Lavie since the first inspection on file was conducted in October of 2021.
